Painful lumbar spinal disorders represent a leading cause of disability in the US and worldwide. Interventional treatments for lumbar disorders are an effective treatment for the pain and disability from low back pain. The American Society of Pain and Neuroscience (ASPN) identified an educational need for a comprehensive clinical guideline to provide evidence-based recommendations. As a result, experts from the fields of Anesthesiology, Physiatry, Neurology, Neurosurgery, Radiology, and Pain Psychology developed the ASPN Back Guideline.
The ASPN Back Guideline represents the first comprehensive analysis and grading of the existing and emerging interventional treatments available for low back pain in relation to injection therapy, minimally invasive spine procedures, neuromodulation, and ablative therapies. Under the category of minimally invasive spine procedures, percutaneous image-guided minimally invasive lumbar decompression (PILD), commonly referred to as the mild® Procedure, demonstrated sufficient evidence in the form of three randomized controlled trials (RCTs) and several prospective observational studies, for the committee to make recommendations.
The guideline provides a comprehensive framework for the safe and effective utilization of interventional treatments for lumbar disorders and concludes that treatments should be utilized based on the published efficacy and relative risk data.

Treatment options shown are commonly offered once conservative therapies (e.g., physical therapy, pain medications, chiropractic) are not providing adequate relief. This is not intended to be a complete list of all treatments available. Doctors typically recommend treatments based on their safety profile, typically prioritizing low risk/less aggressive procedures before higher risk/more aggressive procedures, but will determine which treatments are appropriate for their patients.
Although the complication rate for the mild® Procedure is low, as with most surgical procedures, serious adverse events, some of which can be fatal, can occur, including heart attack, cardiac arrest (heart stops beating), stroke, and embolism (blood or fat that migrates to the lungs or heart). Other risks include infection and bleeding, spinal cord and nerve injury that can, in rare instances, cause paralysis. This procedure is not for everyone.
